Parent or Guardian,

            Medications that have been in the nurse’s office during the school year must be either picked up or will be sent home with student the last week of school. Medication cannot be left at school over the summer. If you plan on sending medications for students back with them at the beginning of the school year they must be accompanied by the new AVERY INDEPENDENT SCHOOL DISTRICT MEDICATION REQUEST FORM. This form will be sent home with the health update form at the beginning of the school year and will be available on the Avery ISD website under the Nurse’s Corner.

            Those students entering into Kindergarten and Seventh Grade Next Year were sent home with a note stating that they may need vaccinations for the upcoming school year. The summer is a great time to take your child in for a checkup and have them receive any vaccinations at that time. Make sure that the school has updated shot records for your child prior to school starting. The State of Texas says that a child is not to attend school if a child does not have required vaccinations or an Exemption on file.

            If your child was referred for their hearing or vision the summer is also a good time to make these appointments if they have not already been seen.  If you have any questions you can contact the school nurse at 903-684-3431.
